What's with the extra holes (closest to the tree) and the circular cutout at the V?  I like it - just curious.

The 5/8" or 3/4" AC plywood would hold you just fine. I would upgrade the rope to maybe 3/4" diameter to cut down on stretch. You also might want to put some 1.5 " wide pieces of flat steel with " teeth " ground in them like say, the teeth on Summit stands to help bite the tree and eliminate any slippage.

The dimensions are 14x18 in the v and the circular cutout in the v is just because it looks good.
