want a simple card holder that dosen't flap around so I am thinking of using a pocket quiver that slip on the belt,

anyone tried this if  so could you tell me who made your quiver

tried to locate some here around Montreal but no success

and if you know someone on the site who make them I would like a place for the pen added to it

I think 3rivers has pocket quivers? If you want to spend for a real "forever" quiver I would suggest Mike's Leather. He is a sponser here. He could probably make one to suit you.
I used to make the holster type side quivers with a pencil slot and a place behind the accessories pocket to put the card. I basically copied the old King quiver.

take a paper spring clip and attach it to your quiver.  my side quiver was made with a card pocket on the side.
