Cityboy doctor or not, get it treated NOW.  Get a physical therapist.  See a rheumatologist.  Get professional second opinions about root causes and treatments.  If it means giving up shooting for 6 months -- DO IT.  The alternative may be giving up archery forever -- and that's a LONG time when you're 38!

I developed degenerative osteoarthritis sometime before I was 12, as when I was diagnosed at 12 it was already severe.  Try thumb release-I can't use split or three under because of hand damage, but I can pull near 100# thumb style.

It still limits my practice sessions if I shoot my heaviest bows, but I can shoot 50# all day.  In fact, my arthritis flares up for a few days, then get better as I get the joints moving.

As some have mentioned, lifestyle has a lot to do with it.  I never have soda-pop drinks or cola or sugar of any kind  - they/it make your system very acidic, which leads to arthritis.  Have a warm lemon drink every morning (very alkalizing), lots of fruit and veg, drink lots of water every day.  Not too much meat (acidic again).  I have taken glucosamine for years - which is a great supplement for joints.  I have a wrist that was badly smashed 25yrs ago, and doctors told me I would be in all sorts of strife for the rest of my life.  It looks crooked and terrible, but is just as strong and useful as my 'good' wrist.  If I hadn't followed such a good lifestyle for all of my life, I would be in deep p?? now.  I am 58.

One hour of life, crowded to the full with glorious action .... is worth whole years of those mean observances of paltry decorum, in which men steal through existence, like sluggish waters through a marsh, without either honor or observation

Sutto, all I can say is, you can lead a horse to water and, you know the rest !

Left wrist got arty in the Trapezium joint. Am 57 shoot 50#, sometimes good...sometimes hurts. Saw one quack that wanted to cut this and cut that, move a tendon. I said whoa...not happening I will just deal with it. So far I have. Also got stenosis of the spine in the neck, I just keep moving. Good luck.
