Ok, I fixed everything.  In the end the easiest thing to do was to create a webpage for the form and database it can be found at the following link.

If you would like to enter data please do.  Please keep answers short, free of special characters and units are not necessary.  If people participate this should prove very useful to all of us.  After the form is finished google will produce some charts and graphs with the data on hand to summarize the previous responses.  It is pretty slick for very little effort on my part.  Thanks to this thread for giving me something to obsess over tonight.  

As for modifications to the form, we should have a discussion and see what you want on your form and what you want from the data.

Cherokee- works fine, just use cursor arrows and scroll it over.

Josh-  This is AWESOME! Thanks.  This could really become a VERY useful tool.  Another resource that makes Tradgang a pretty great site.

A couple thoughts:  Could instructions for the survey explain to fill in all boxes- even with NA if they like?  Could we make the species we hunt check as many as apply?  

Moderators- can we put this in a prominent/useable place? Is that appropriate??
