Any suggestions as to who would do a good job crafting a custom made tab?

Have you tried Scott Teaschner at Wyoming Wildlife Artistry. I don't know about a tab, but he does excellent work.
What do you mean by a custom made tab? Are you thinking tooled leather or just custom made to your size?

Rod Jenkins, a sponsor here, carries Scott Treashner's tabs. I like them and they are my main tab now. However, tabs are very personal, so YMMV. I don't know if Scott makes custom tabs to your specs or not.

I'm not sure what the term "tooled leather" means, but what I am looking for is someone who can take an idea I have for a tab and actually bring it to life. My idea incorporates the best features of a couple different types of tabs that I have used over the years.

Tooled leather is basically what Scott Teaschner does on his armguards. Basket weave stamping and any borders or any impression in leather. It doesn't sound like what you were referring too anyway.
I seen this post and just wanted to comment since I seen my name mentioned. First I want to thank everyone who promotes me on this site. This Site was the main catapult to my success! With that success comes inherent problems. Like how does one man keep up with everything? Just getting Rods tab in production has been a process.
I try do design what I call "High Grade Gear" I absorb the cost of research and development over a period of time. This is a way to offer customers a real quality product with out having to reinvent the wheel every time. Of course some things can be customized to personal taste like tooling patterns, colors or initials. What I have may not please every one.
What may be what some think simple still seems to take at least a few hours minimum and most likely longer. I do not know how to answer this problem one hates to turn away work but I have to make money or I will not be around to provide my services.
I have learned through other "custom orders" where the person has a idea and I am their hands it eats up a bunch of time. So I come up with a plan. I will do these jobs but this is how it will work. I keep track of my hours phone calls, sending pictures, mock ups all this time is kept track of. Then in the end when all the time and materials are added up that's the cost. I will also require a deposit to start then project.
When I build a tab in quantities they may retail for $15 to $30. But when I have to do a one of a kind to some ones specs it may end up being $75 to a $100. Now some may say that is ridiculous and it may be but time is money.
I just wanted to clear this up because as time goes on I get more and more people wanting me to be their hands in some idea they have.

I just bought two of Rod Jenkins' tabs at Safari Tuff. I think they are made for him by Far West?
These are terrific tabs for a 3-under shooter. They are a lot like the Cavalier Elite without the metal lower finger piece.
I did find that I use a Small in the Cavalier and had to order Medium from safari tuff. They are very well made by the way.
I'm curious how it will hold up. I shoot daily. I've shot the dickens out of the Cavalier for well more than a year with no problems. I like all-leather Safari Tuff because there is nothing on the tab that can scratch my bow.

Nice Tabs but a mine seemed to be a tad short. A little longer sure would be nice.
I make the tab for Rod Jenkins and my business name is Wyoming Wildlife Artistry.
We are the only ones selling them at this point. The tab is designed for a deep hook on the string. With that being said we figured in the length with that in mind.
I might be interested in a tab also can I get a website so I can look??

Scott, you describe a normal business process. No one should complain after your time is invested in a single product. Most of us understand ROI, and the cost of R&D is legit
I have one of Scott's O-Ring tabs and it is fantastic, I also have one of his arm guards and it is in my opinion the Roll Royce of Arm guards - faultless.
I really like the look of the one he has done for Safari Tuff, If it was for split instead of 3 under I would jump at it, no hesitation.
I HAVE AN EAGLES FLIGHT CORDOVAN TAB VERY NICE.
I have 4 of Scott's O-ring tabs. They are the best. I also like the looks of the tab he makes for Rod, and it would be nice in a split finger, but the O-ring tab is his split finger tab and it works very well. All of Scott's products are outstanding and well worth what he charges. Besides, Scott is a great guy.
