Who makes the foam disks for aerial shooting that Kustom King and 3 Rivers sell?

You could try giving 3Rivers a call, they should be able to tell you

better yet make your own. Just use a cardboard box and trace several circles the same size(i.e. tupperware bowl ,paper plate,coffecan)cut them out duct tape 5- 7 of them together an have fun.They are heavier than foam ones so you can throw them better in windy conditions.Hope this helps

We buy the square foam broadhead Target from Wal-Mart and cut a circle from it. Works great.

you can also use craft foam and duct tape

That's what I did, only I didn't buy it just for that purpose. I had used it for broadheads and decided to cut me a circle out of it after many years of seeing it just sitting in the shed. Works great and has some heft to it as well.
